How do people living outside the USA get access to Uru Live?

More easily than here! :P

Outside of the US and Canada, I believe that other countries will have access to Uru Live without the Gametap front door. The downside is that you'll still have to pay the same price as if you were getting all of Gametap.

GameTap is now on a rolling release outside of the US, with Canada a few days ago and a set of countries including the UK in the new year.

The short answer to your question is that as of now we don't know.

This is what GameTap wrote me on 27th of October:

It doesn't seem as though the whole of GameTap is awake yet. Canada went live on 17th October.

I have found that GameTap Customer Service representatives are friendly and eager to help, but are famously uninformed with many having not heard of Uru Live, some having not heard of Cyan Worlds, and even a supervisor mistakenly and specifically confirming the launch of Uru Live last month.
